Description of Service:

Siam Medical Travel, LLC’s (“Siam Medical Travel”) purpose is to assist individuals and companies identify medical providers with regard to medical, dental and/or cosmetic treatments and recuperation packages for destinations on an international basis. In our role as a medical travel facilitator, we research and compile information regarding healthcare and cosmetic surgery providers and offer this information for personal use in making a more informed decision about obtaining medical, dental and cosmetic treatment both outside and inside the United States.

Siam Medical Travel is not a medical service provider, nor is it a medical referral service, and it does not endorse, recommend, or approve any healthcare provider listed in the Site or otherwise noted either verbally or in writing.  We are not medical professionals and will not discuss or advise on any issues relating to medical treatment.
We facilitate contact with, and provide information about physicians and healthcare providers outside the United States, and any accreditation and credentials are provided by those providers and are not necessarily recognized or endorsed by any United States governmental or professional agency.  Siam Medical Travel does not conduct any research or authentication of any accreditation or credentials. This information is supplied so Siam Medical Travel clients can make decisions based on the information provided by the hospitals and their medical staff.  
While Siam Medical Travel provides information it does not recommend any preferred or specific doctors for your desired medical treatment.  We look to offer our clients options which will allow for informed decision making. You should know that we may receive a referral fee from the provider and/or facilities, but that does not mean nor should it be construed that receipt of any referral fee means that we in any way warranty, guarantee or assert any medical outcome.

We recommend that to the extent possible all Siam Medical Travel clients discuss all medical issues with your medical doctor in the United States before making a decision on medical treatment outside the United States.

All decisions are yours alone, and we do not endorse any medical provider or facility, nor do we make any representation that services offered by any medical provider or facility are suitable for your specific conditions, treatments or other medical related attributes.  It is solely your responsible to determine the qualifications, credentials, services of the medical provider and/or facility.

Prices:

All prices provided on the Site or in our brochures and associated materials are subject to change without notice and should be interpreted as estimates. Airlines, hotels and medical service providers may change their prices at any time.  Currencies fluctuate on a daily basis while geopolitical factors also contribute to price changes.

Special conditions may apply to the prices set out in our materials and on the Site. To determine such conditions, you must contact Siam Medical Travel.

The prices set out in our materials and on the Site may not include taxes or airport charges.

Payment Terms:

All clients will be required to make payment through certified bank check or credit card. We do not receive payments on behalf of medical providers or facilities.  We do require payment of a fee for the services we provide, via a variety of payment options.  Depending on the country and medical provider, payment maybe required at the time of booking or at the time of delivery of services.  Different medical service providers have different billing procedures.  Total payment may include but are not limited to hospital expenses, procedure price, airfare, hotel, and ground transportation.

Cancellation Fees:

Should a cancellation occur transportation, accommodation and medical service providers and other third parties may impose cancellation fees. You agree you will be solely responsible for payment of any and all such fees and that Siam Medical Travel will have no liability for any such fees. To protect against travel cancellation charges and potential medical malpractice, we strongly recommend purchasing Cancellation Protection, Travel Interruption and medical malpractice Insurance should it be available.

Limitation of Liability:

In no event shall Siam Medical Travel be liable for ant indirect, incidental, special, punitive or consequential damages (including damages for loss of business, profit, revenue, data, use or other economic advantage) incurred by any distributor or any other third party, in connection with or arising from the use of any of the services provided and in association with this site, however it may arise, whether in an action in contract or tort.

Disclaimer of Warranties:

Siam Medical Travel makes no warranty, expressed or implied, regarding the fitness of the services provided by any third party,  these include but are not limited to hospitals, healthcare providers, physicians, hotels, and transportation providers.

Arbitration:

Any dispute or claim arising under or with respect to this user agreement will be resolved by arbitration under the provisions of the Massachusetts Arbitration Act, the parties shall be responsible for payment of their own expenses and attorney’s fees.  Use of this service is voluntary, and thus, you specifically agree that all disputes will be governed by Arbitration in the Commonwealth of Massachusetts.

Privacy Policy

Siam Medical Travel respects your privacy. The purpose of this Privacy Policy is to outline how we deal with any personal data you provide to us while visiting our Site.

By visiting this website, you are accepting the terms of this Privacy Policy. Any external links to other websites are clearly identifiable as such, and we are not responsible for the content or the privacy policies of these other sites.

Information Collected:

We keep two types of information relating to visitors to our website:

“Personal Data”: This is data that identifies you or can be used to identify or contact you and may include your name, postal address, email address, telephone number and birth date. Such information is only collected from you if you voluntarily submit it to us, for example, through our “Contact Us” email facility or by completing one of our online forms.
“Non-Personal Data”: Like most websites, we gather statistical and other analytical information collected on an aggregate basis of all visitors to our site. This Non-Personal Data comprises information that cannot be used to identify or contact you, such as demographic information regarding, for example, user IP addresses, browser types and other anonymous statistical data involving the use of our Site.

Purposes for which we hold your Information.

Non-Personal Data: We use the Non-Personal Data gathered from visitors to our website in an aggregate form to get a better understanding of where our visitors come from and to help us better design and organize our website.

Personal Data Including the Release of Medical Information

We may process any Personal Data you provide to us for the following purposes:

    (a) to provide you with information about the services of medical providers and facilities;
    (b) to contact you if required in connection with your enquiry or to respond to any     communications you might send to us; and/or
     (c) to pass along any medical information you voluntarily provide to us to medical providers and facilities.

By providing us with any personal medical information, you specifically grant us permission to transmit that date to medical providers and facilities. By agreeing to these terms, you specifically grant us permission to disclosure your medical information to the medical providers and facilities, and explicity grant us that right with regard to any HIV/AIDS information, mental conditions or treatments, drug counseling or rehabilitation, all as defined with the General Laws of Massachusetts, the Federal Statutes.

Any communications submitted online or by email will be subject to the same terms and conditions as those completed by hand in paper form.

Disclosure of Information to Third Parties

We may provide Non-Personal Data to third parties, where such information is combined with similar information of other users of our Site. For example, we might inform third parties regarding the number of unique users who visit our Site, the demographic breakdown of our community users of our Site, or the activities that visitors to our Site engage in while on our Site. The third parties to whom we may provide this information may include our web site design, development and hosting contractors.

We will not disclose your Personal Data to third parties outside of the company unless you have consented to this disclosure or unless the disclosure is required in order to process an application or other communication from you or to perform the services you have applied for. A request for communication with a doctor is considered consent to release Personal Data. 

We will disclose your Personal Data if we believe in good faith that we are required to disclose it in order to comply with any applicable law, a summons, a search warrant, a court or regulatory order, or other valid legal process.

Sale of Business

We reserve the right to transfer information (including your Personal Data) to a third party in the event of a sale, merger, liquidation, receivership or transfer of all or substantially all of the assets of our company provided that the third party agrees to adhere to the terms of the Privacy Policy.

Security

Your Personal Data is held on secure servers hosted by our Internet Service Provider. Unfortunately, no data transmission over the Internet can be guaranteed to be 100% secure. As a result, while we will take all reasonable steps to protect your Personal Data, the nature of the Internet is such that we cannot guarantee or warrant the security of any information you transmit to us via the Internet.

Updating, Verifying and Deleting Personal Data

You may inform us of any changes in your Personal Data we will update or delete your Personal Data accordingly. Any such data subject requests may be subject to a processing fee.

Changes to the Privacy Policy

Any changes to this Privacy Policy will be posted on the Site so you are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. If at any time we decide to use Personal Data in a manner significantly different from that stated in this Privacy Policy, or otherwise disclosed to you at the time it was collected, we will notify you by email, and you will have a choice as to whether or not we use your information in the new manner.
